Consultation developing project on TDM(Therapeutic Drug Monitoring) through a survey of residents

Abstract
TDM(Therapeutic Drug Monitoring) is an information supporting process to give optimal therapy by both maximizing drug efficiency and minimizing toxicity with plasma level monitoring. It is for the plasma level monitoring required drug because of narrow therapeutic range and individual drug dynamic differences. This system may help setting patient oriented individualized dosage level through monitoring not only drug level by time but also response on drug therapy.£±£¬£²£©In our department, we have been doing TDM(Therapeutic Drug Monitoring) consultation on 3 drugs including Theophylline, Vancomycin and Digoxin in November, 2007. In this study, we research on TDM consultation program awareness and improvement compared with current and previous number of consulting requests to search any improvements needed and continuously develop this program. We collected 56 survey sheets from 70 residents in 11 departments that are related TDM drugs. The result showed that there were very low awareness of TDM program among residents. In the result, 57% of residents were not aware of TDM program, 75% did not know how to request consultation and 75% did not know how to look for consultation process system. Also, because they did not request consultation, 50% did not know how to use, 16% did not have a drug in the list, 78% answered they would use when drugs are added. After this research we needed to increase residents¡¯ awareness of this program, we delivered PR materials on TDM objectives and method for residents on 17th August. Also, we made manual and sorted information on the drug which resident needs practical consultation and added valproic acid and carbamazepine TDM on 21 September. To improve consultation quality and diversity of drugs in the list by strengthening pharmacists ability, we participated special congress and workshops conducted by KSHP and studied in Clinical Pharmacokinetics Course. In results, average monthly number of consultation requests increased from 3 during Jan and July to 13.25 during Aug~Sep in 2007.
